Position Summary/Essential Duties:

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans.  Serves a resource to others in work-team and department.

  1. Provides psychosocial assessment / intervention to assist patients/families (including clinically complex)
    • Provides age specific care to patients/families in all age groups.
  2. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
    • Develops plans with attention to individual patient and family goals
    • Provides linkage to community resources and UH network services to facilitate optimal care
  3. Communicates/collaborates with the work team using a multidisciplinary approach.
  4. Provides guidance/consultation to peers/team regarding routine psychosocial issues and /or discharge planning issues.

Position Requirements:

  • Two (2) years’ experience, in a hospital setting desirable
  • Computer skills
  • Theoretical and clinical knowledge regarding health care delivery
  • Multi-tasks and prioritizes work
  • Communicates effectively with persons of various backgrounds
  • Demonstrates strong interpersonal skills and excellent communication skills, both written and verbal
  • Exhibits strong organizational skills and work ethic
  • Demonstrates a professional demeanor when responding to patients/customers

 

Education/Expertise:

Bachelor’s degree from an accredited school of Social Work

 

Required License:

Licensed by the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board at the (LSW level)

 

Preferred Qualifications/Position Details:

Previous work with the pediatric population is strongly preferred.  The position will work with those that are victims of gang-related and community violence.  Travel within the local communities and schools are required.  Schedule is Monday-Friday, 8:00am-4:30pm. This is a 1 year grant-funded position.
