 Case Management Department Manager

The Case Management Manager provides leadership to the staff and oversees daily

functions of the department. Responsibilities include utilization review, care coordination,

discharge planning & resource management based on patient assessment and needs after

discharge or transfer; ensures departmental compliance with CMS Conditions of

Participation and TJC’s Provision of Care.

Job Requirements:

• Associates Degree in related field (Bachelors preferred)
• Current GA RN License & BLS (AHA)
• 2 years experience in Case Management
• 2 years of progressive leadership
