Licensed Practical Nurse – Adult Outpatient Behavioral Health

What you will do:

  • Review charts of patients scheduled for the day.
  • Prepares lab slips and checks previous results. Explains lab procedures and reason for tests to patients.
  • Writes, updates, and reviews treatment plans with patients.
  • Answers all medline calls regarding medication and concerns from patients.
  • Does patient teaching regarding medications as well as diagnosis, lifestyle (health issues, diet, exercise) and reinforces teaching done by physician.
  • Assess patients' mental status each visit, observes for change in symptoms as well as uses different scales such as AIMS to assess for medication side effects.
  • Acts as advocate for patient with outside agencies and with physicians.
  • Makes referrals to a different level of care when clinically indicated.
  • Advocates for the patient with the pharmaceutical companies for patient assistance when financially unable to afford medication.
  • Interfaces with internal and external agencies to help provide better continuity of care for patient.
  • ls available as a resource to non-medical staff for education regarding medication and medical concerns.
  • Completes POA's for medications with insurance companies.
  • Is called upon by other therapists to evaluate medical emergencies that occur in the facility and do the follow-up documentation required. Participates in interdisciplinary staffing on new and existing cases.

Responsibilities:

  • Ensures quality nursing care is rendered to all patients in accordance with the New York Nurse Practice Act, National Standards of Practice, and Nursing and Clinical Service standards of care and practice.
  • Utilizing the Nursing Process, plans, directs, and supervises the care of patients in addition to being involved in the provision of direct care of patients and families.

Education, Training, Experience, Certification and Licensure:

  • Graduation from an accredited school for Licensed Practical Nursing and currently licensed in the State of New York.
  • Maintains current BLS/CPR.
  • Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of expertise.

Physical Demands:

  • Medium work: must be able to lift 25 pounds frequently; occasionally lifting maximum of 50 pounds. Will require ability to stand, walk, sit, bend, twist, squat, and reach.

  • All services, medical staff, patients, visitors, and various regulatory and professional agencies.

Supervised By:

  • Team Leader, Clinical Coordinator, Unit Manager, and Clinical Services/Nursing Administration.
