Join our team of competent and compassionate professionals as a Transition Coordinator in the Behavioral Health Community Partner (BHCP) program at Behavioral Health Network!
The Transition Coordinator for the BHCP program works as part of the integrated care management team to coordinate and implement strategies to ensure quality and efficient post-acute care transitions for the population served. Through coordination with local area hospitals, detoxification, and other acute levels of care, the Transition Coordinator assists members in connecting to supports and resources upon discharge to improve their health outcomes and address social determinant of health needs.
The Integration program is an innovative model of care, working directly with primary care and pediatric providers to address the whole health needs of the families and individuals in our culturally diverse community. Our staff includes psychologists, social workers, counselors, recovery coaches and care coordinators. The Integrated Care program is person-centered, evidence-based, and focused on collaborative goal setting to address unmet medical, behavioral mental and social needs.
**NOTICE**
In accordance with the BHN Mandatory COVID vaccine policy - all staff, interns, temps, residents and contractors are required to be fully vaccinated no later than day one of employment. Medical or religious exemptions will be offered and must be approved prior to starting work.
The same guidelines noted above apply during flu season (defined as December 1st – through end of flu season as announced by the CDC) for BHN’s Mandatory Influenza Vaccine policy.
