Dalton Medical Associates, LLP is a well-established independently owned primary care practice. We are seeking a qualified full-time (4 days) Adult Nurse Practitioner/Physician's Assistant to deliver care to our adult population. The candidate will provide comprehensive primary care services and bring a patient-centered mindset in an outpatient setting. The practice has been a leader in the community for many decades and participates in pay for performance initiatives which reward providers based on patient outcomes. Qualifications: Experience in treating a patient panel age 18 + with multiple chronic conditions. A strong commitment to population health and evidence based medicine. Excellent communication skills and a delivery style that is welcoming and open with patients. Willingness to participate in all ACO projects and strive to exceed quality goals. Experience charting in electronic health record (EHR) systems. A strong commitment to timely and accurate completion of patient progress notes. A strong commitment to working in a team based environment.
