Direct Support Professional - Community Engagement
$1000 Sign On Bonus at 6 months of employment!
This is a full-time position with some evenings and weekends. 40 hours per week.
The Community Engagement DSP will provide meaningful person-centered opportunities through planned community engagements. Plan and execute opportunities for clients to access their community through volunteer work and community exploration. Maintain/build community relationships, while encouraging client independence. Provide training in appropriate work expectations while teaching socialization and adaptive skills when necessary.
Community Engagement Direct Support Professional duties include but are not limited to:
- Conducting client orientation to their local community.
- Identifying client needs and assess skill levels.
- Helping clients solve problems and promote independence.
- Assisting clients with personal care as needed.
- Planning and supervising community integrated activities.
- Participating in implementing the clients’ Individual Service Plans (ISPs) and Behavior Plans.
- Advocating and protecting clients’ health, safety, and financial well-being by implementing emergency, safety, and infection control plans.
- Providing clients with information on personal choice options throughout the day.
- Developing, executing, and tracking community engagements and volunteer work.
- Tracking progress in employable skills.
- Monitoring and recording client production and attendance to assure payroll and personal choice data is accurate.
- Maintaining a cooperative relationship with clients, other departments, and the local community.
- Implementing behavior management techniques.
- Backfilling on OHI work floor as needed.
- Planning and supervising contingency activities when community engagement or volunteer work is unavailable.

Opportunity House will provide you with all the necessary training to succeed as a Direct Support Professional! DSP training is provided online and is flexible and paid! Once you successfully complete DSP training, you will be added to the Illinois Healthcare Workers Registry as a DSP as well!

-Review and manage departmental spending; coordinate maintenance and repairs.
-Advocate and protect clients’ health, safety, and financial well-being; ensure clients receive all services and benefits to which they are entitled.

-Responsible for on-call scheduling issues, emergencies, and other emergency situations that require a house manager.

Residential Client Support Manager

Summary
The Residential Client Support Manager provides direct supervision of all Residential QIDPs/Case Managers to ensure quality direct care services to clients served, by overseeing medical, behavioral, and all entitlements. Residential Client Support Manager collaboratively works with Residential Director and Assistant Director to maintain client care needs and to ensure all residential sites remain in compliance with licensing and regulatory compliance.
Essential Functions
-Use independent judgment and decision-making skills to work independently.
-Oversee Residential caseloads as needed to ensure continuity of client care, benefits, and entitlements are maintained.-Participate in scheduled house meetings, client staffing, Behavior Management Committee meetings, and all other scheduled client and/or agency meetings.
-Communicate effectively with clients, staff, the Residential Director, and other members of the interdisciplinary team.
-Advocate for the client’s health, safety, welfare, and personal well-being by promoting safety procedures and communicating client health or safety concerns with managers.
-Comply with applicable work rules, policies, procedures, accrediting, and licensing regulations.
-Serve as Behavior Management Committee Co-Chairperson and assist the Chairperson with any duties and responsibilities that she may have to complete for the committee.
-Serve as an active member of any task force or committee that focuses on case management and or the development of agency policy and procedures.
-Serve as Co-chair on Case Management Task Force and all other committees that may or may not be delegated.
-Serve as an active member of any task force or committee that focuses on the development of the agency.
-Assist with psych appointments and client medical appointments if a Case Manager is absent or the caseload is open.
-Assist Case Managers with On-call Health and Behavior emergencies as needed.
-Maintain on-call schedule for the Residential program, including filling in any gaps in the on-call schedule, responding to any last-minute on-call manager call-offs; be willing to cover any openings that still need to be filled.
-Communicate bi-weekly with Behavior Support Analysts and make sure that all needed Therap data information is updated according to their reports.
-Communicate effectively with our Nursing Department to ensure all clients’ medical needs are being met.
-Be an authorized QIDP Trainer with the DHS within 120 days of hire.
-Conduct bi-weekly Case manager meetings with the members of your team to keep open communication and work towards goal completion.
-Participate in Med Authorization Class and become medication authorized in all residential homes within 120 days of hire.
-Assist with medication passes, open shifts, client pick-ups, and drop-offs as your schedule allows.
-Complete five quarterly residential safety inspections.
-Other duties as assigned.

-Interview, recruit, and hire staff; appraise work performance and provide job counsel.

Food Service Director

Provides adequate meal preparation to residents living in a Community Living Facility by actively working with nutritionist to create balanced meals for all clients. Will be responsible for completing Direct Support Professional tasks while essentially catering to client’s health and medical needs.
Essential Functions
• Obtain menu approval from the registered dietician consultant and plan menus.
• Cook nutritious and tasty meals according to the menus.
• Purchase food and store according to sanitation regulations.
• Manage purchasing to stay within specified food and household budgets.
• Complete a weekly inventory of food stocks.
• Make sure kitchen, dining area, and storage area are cleaned properly.
• Supervise and assist clients involved in programming.
• Assist clients in solving problems related to provided services.
• Learn and implement behavior intervention techniques.
• Evaluate the efficiency and effectiveness of the food service operation and adjust as necessary.
• Assist with client medical appointments as needed.
• Complete grocery budgeting for clients actively receiving snap benefits monthly.
• Administer medications to clients as necessary.
• Other duties as assigned.
