Join our team of competent and compassionate professionals as a Direct Care – Outreach Support Specialist in the Individual & Community Supports program at Behavioral Health Network! 

 

**NOTICE**

In accordance with the BHN Mandatory COVID vaccine policy - all staff, interns, temps, residents and contractors are required to be fully vaccinated no later than day one of employment. Medical or religious exemptions will be offered and must be approved prior to starting work.

Your experience, knowledge and great skills will support the Individual & Community Supports program with the following:

  • Providing direct services that support growth, promote independence, and ensure the safety of adults with intellectual disabilities living independently in the community.
  • Assisting individuals with household management tasks such as sorting mail, paying bills, organizing appointments, communicating with landlords, and completing household chores.
  • Helping individuals maintain optimal health by ensuring routine and medical care as needed, encouraging compliance with treatment recommendations, and supporting access to community providers.
  • Providing individuals with opportunities for community integration and social support.
  • Implementing strategies for achieving support plan goals.
  • Promoting individuals’ abilities to live independently in the community.
  • Completing service notes and other required documentation in a thorough and timely manner.
  • Effectively communicating with case managers regarding individuals’ service needs.
  • Providing safe and timely transportation in own vehicle.
  • Recognizing and immediately reporting housing, health, or other safety concerns to supervisor.

Funded by Department of Developmental Services (DDS), the Individual & Community Living Supports program at BHN provides hourly supports to adults with developmental or intellectual disabilities living on their own in the community. The ultimate goal is to help the individual live on their own safely and be as productive in the community as their skills allow them to be.

Job Requirements:

What are we looking for in a Direct Care – Outreach Support Specialist?

  • Commitment to empowerment and growth, human rights, and community integration of persons served (required).
  • At least six months of experience in human services (preferred).
  • High school diploma or G.E.D. equivalent (required).
  • Bilingual in Spanish & English (preferred).
  • Valid driver's license and a reliable car available for working hours (required).
  • Proficiency with written and oral communication (required).
  • Familiarity with computers (preferred).
  • Training is provided by BHN.
