Social Worker
Responsible for the overall management of all social work services for assigned home care clients. Under the direction of the client’s primary care physician and Hospice Medical Director, plans and provides individualized social work intervention to enable clients to maintain dignity, independence and choice during the end stage of life. Participates as a member of the IDT to plan and provide holistic care to patients and families. Interventions evolve from ongoing psychosocial assessment and are in accordance with the IDT Care Plan.
Desired Qualification:
RNs
Under the direction of client’s primary care physician and the case managing or on-call RN, the staff nurse provides intermittent skilled nursing visits. Visits may be scheduled through advance notice or may be performed to respond to emergent needs. Under special circumstances as directed by the DNS, may perform hourly skilled nursing care in the client’s home setting. Participates as a member of the Interdisciplinary Team (IDT) to plan and provide holistic care to patients and families. Nursing care reflects observations, actions, and plans that are in accordance with the approved plan of care (POC). Must be flexible in work hours for staffing purposes.
Qualifications:
-Current State of Washington RN license in good standing. BLS for Health Care Providers certification required.
-Valid driver’s license, acceptable driving record, personal vehicle for on-the-job transportation.
NACs
Qualifications:
-Current State of Washington NAC certification in good standing.
-Current Basic Life Saving for Health Care Providers (CPR and AED) certification required.
-Food Handler’s permit
-Valid driver’s license, acceptable driving record and personal vehicle for on-the-job transportation, evidence of insurance coverage as required by the State of Washington.
