Social Worker

Responsible for the overall management of all social work services for assigned home care clients. Under the direction of the client’s primary care physician and Hospice Medical Director, plans and provides individualized social work intervention to enable clients to maintain dignity, independence and choice during the end stage of life. Participates as a member of the IDT to plan and provide holistic care to patients and families. Interventions evolve from ongoing psychosocial assessment and are in accordance with the IDT Care Plan.

Desired Qualification:

  1. Master’s Degree in Social Work and one year professional social work experience in healthcare environment or Bachelor’s in Social Work with applicable experience.
  2. Necessary license or certification to practice as a social worker in the State of Washington. Current Basic Life Saving for Health Care Providers (CPR and AED) certification required.
  3. One year professional social work experience required in a healthcare environment.
  4.  A valid driver’s license, acceptable driving record and personal vehicle for on-the-job transportation with evidence of auto insurance coverage as required by the State of Washington.

RNs

Under the direction of client’s primary care physician and the case managing or on-call RN, the staff nurse provides intermittent skilled nursing visits. Visits may be scheduled through advance notice or may be performed to respond to emergent needs. Under special circumstances as directed by the DNS, may perform hourly skilled nursing care in the client’s home setting. Participates as a member of the Interdisciplinary Team (IDT) to plan and provide holistic care to patients and families. Nursing care reflects observations, actions, and plans that are in accordance with the approved plan of care (POC). Must be flexible in work hours for staffing purposes.

Qualifications:

-Current State of Washington RN license in good standing. BLS for Health Care Providers certification required.

-Valid driver’s license, acceptable driving record, personal vehicle for on-the-job transportation.

NACs

  1. Provides services at homes and/or facilities according to the Hospice Aide Plan of Care. Follows standard precautions for infection control.
  2. Maintains a clean and safe environment in the client’s area.
  3. Provides to the client appropriate and safe personal hygiene and grooming.
  4. Provides the client safe transfer and ambulation as directed.
  5. Performs household services per Care Plan.
  6. Completes appropriate documentation in a timely manner.

Qualifications:

-Current State of Washington NAC certification in good standing.

-Current Basic Life Saving for Health Care Providers (CPR and AED) certification required.

-Food Handler’s permit

-Valid driver’s license, acceptable driving record and personal vehicle for on-the-job transportation, evidence of insurance coverage as required by the State of Washington.
