POSITION PURPOSE

The Palliative Care Program Coordinatior position supports the Project “COMPASSION” (Center of Medical, Palliative, and Spiritual Support in Our Neighbohoods) grant awarded by the Mother Cabrini Health Foundation for the 2022 calendar year. This position is primarily responsible for assisting the Program Director in delivering on the grant’s activities, goals and outcomes by supporting the creation/production of training and educational programs, a Project “COMPASSION” website, and the promotion of lay minister training. The Program Coordinator also manages and tracks the implementation milestones and achievement of ongoing activities to help ensure the success of this important initiative.

ESSENTIAL FUNCTIONS

Knows, understands, incorporates and demonstrates the mission, vision and values of Trinity Health and St. Joseph’s Health in leadership behaviors, practices and decisions

Supports all Program communication, work plan, and meeting activities to ensure the project runs smoothly and efficiently

Serves as professional, reliable and trusted point of contact for internal and external constituencies on any items related to project coordination

Acts on communications (emails, leadership meeting materials, etc.) while partnering with the broader team to provide administrative guidance to ensure compliance with funder guidelines and to help achieve projected outcomes

Works with stakeholders, including St. Joseph’s Finance and Grants Teams, to assist with the submission of interim progress reports, financial reports, budgets, and work plans

Assists Program Director with information/data management, vendor contract development and organization, invoice processing, and bookkeeping

Assists with the scheduling and planning of training events with community and local faith-based organizations across the designated catchment area

Identifies key issues of importance that need the attention of the Program Director

Works with Project Director on various project tasks as they arise

Completes all mandatory trainings and in-services required by Trinity Health and St. Joseph’s Health

Maintains a working knowledge of applicable Federal, State and local laws/regulations; the Trinity Health Integrity and Compliance Program and Code of Conduct; as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior

Other duties as assigned

MINIMUM QUALIFICATIONS

Prior experience as a Project Coordinator or Manager is required

Previous experience in a health care setting, community based organization, or faith-based ministry is a plus

Ability to manage multiple tasks, be detail oriented, be responsive, and demonstrate independent and innovative thought and critical thinking

Excellent communication (verbal and written) and presentation skills

Experience using Microsoft products such as Word, Excel and PowerPoint preferred

Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the mission, vision, values and goals of Trinity Health

Must have own vehicle and be able to drive locally and statewide (work related mileage to be reimbursed for travel over three miles between points)

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

Must be able to adapt to frequently changing work demands, multi-task, and determine work priorities

Must be able to work in a fast-paced environment and concurrently focus on a variety of complex tasks / projects while also being mindful of interpersonal work styles and personalities

Must be comfortable in an environment that may be stressful due to sensitive topics related to palliative care, end-of-life, spiritual care, etc.

PHYSICAL DEMANDS

Standing, walking, driving a vehicle, lifting light cases of educational and promotional materials. Must be able to lift up to 25 pounds
