Community VNA, a growing, dynamic home health care agency, is seeking a Community Liaison RN. This position will be responsible for generating referrals and coordinating services for referred patients, particularly in institutional settings. Educate and act as a resource to physicians, discharge planners, and case managers. Responsible for marketing all agency programs and services. Working with the business development team, is responsible for driving census growth.
• Represents Community VNA Service lines in assigned hospitals, nursing homes and assisted living facilities
• Cultivates and develops relationships in assigned accounts
• Works in collaboration with intake and referral services to ensure smooth transition to services.
Qualifications: Current Massachusetts RN licensure required: 3-5 years marketing experience; home care/Hospice experience a plus; working knowledge of CRN’s, Play-maker experience helpful; knowledge of Southeastern Massachusetts referral sources required; willingness to travel into Boston 1-2 times per month; data driven.
