$500 Sign on Bonus Available!

The Case Manager Discharge Planner (Intercept 2) provides discharge planning and coordination, comprised of scheduling follow up appointments with BRBH for treatment services, case management, medication evaluations, and other related services for personas with Serious Mental Illness (SMI), including Co-occurring Disorders, being released from the Roanoke City Jail who are either current BRBH clients or eligible for such services through BRBH. (MH Rural Jail Diversion funds/BHD Intercept 2 grant)

Minimum Requirements:

• Education: Bachelor's degree in an acceptable human services field is required.
• Licensure/Certifications: Qualified Mental Health Professional (QMHP) preferred.
• Safe Driving Record: Candidates must have a valid Virginia driver's license with a safe driving record (no more than two moving violations in the last three years and no criminal violations in the last 5 years.)
• Background Check: Candidates that are offered a position are required to undergo a background check and drug screen.
