The Care Manager is responsible for facilitating and coordinating the care delivered to Transitional Care Management(TCM)-eligible patients through multidisciplinary and patient/family collaboration. Coordination involves telephonic outreach, planning, evaluation of patient care related outcomes, and appointment scheduling. Broad clinical background with knowledge and understanding of best practices, managed care, clinical pathways or guidelines, regulatory mandates required. Must have current-RN license to practice in Louisiana.BSN preferred. Current American BLS required. Minimum of 2 years' clinical experience required.
