New Position

COMMUNITY HEALTH NAVIGATOR: Full-time 10-month school year position.

• Coordinate and monitor all aspects of health related student services throughout the K-12 system. (mental/behavioral health, AODA, appointments/assessments, school wide health screenings for vision, dental, hearing, concussion protocols, athletic physicals).
• Works in collaboration with the primary care provider and all members of the healthcare team.
• Assesses healthcare needs and identifies community.
• Coordinate health related community resources.
• Serves as patient advocate and assists in identification and improvement of service delivery.

QUALIFICATIONS:

• Minimum Bachelor’s Degree in Human Services or related field preferred.
• Two years of nursing, social work, counseling or therapeutic work and experience in community Preferred.
• CPR Certification required.
• Experience with utilization review and discharge planning required.

JOB REQUIREMENTS:

• Background investigation, drug test
• Experience working with native children preferred.
• Must be supportive of the philosophy, concept, policy and procedures of the Lac Courte Oreilles Ojibwe-Waadookodaading School.
