- Provides personal care and assists with home care patients, under the supervision of the RN Case Manager or therapist, Initiates and maintains positive relationships with patients customers and co workers.
- Observes and reports patient status changes including skin and psychosocial, takes responsibility for self development and supports a learning environment, displays commitment to the mission of UHHS UHHCS and its values.
- Provides personal care and companionship as directed by plan of care.
- Provides light housekeeping and light meal prep tasks as directed by plan of care, effectively communicates both verbally and in writing.
- Succesful completion of agency written test and competency evaluation of skills in the field within one month of hire and annually.
- Completes required continuing education requirements to maintain certification.
- Full time 40 hours per week.
- This position will cover the far eastside-primarily Geauga & Ashtabula Counties.

- High School Equivalent / GED.
- STNA - State Tested Nursing Assistant.
- Minimum one year home care experience preferred.
- Current CPR certification or obtain within one month of hire.
- Valid drivers license required car insurance coverage.
