Lead Care Coordinator

Lead Care Coordinators provide telephonic and in-person Care Coordination based on sound theories and principals. This includes all appropriate members in the individual’s Integrated Care Team, including primary care providers, therapists, and others as indicated by the person served.
Lead Care Coordinators monitor the effectiveness of the Integrated Care Plans and coordinate with care team to make any changes as needed.

Job Requirements:
Lead Care Coordinators at Behavioral Health Network have the following qualities:
- 6 months of work experience in a related behavioral health, human services and/or medical field (required)
- Knowledge of substance abuse, mental illness, and the interface between mental health and medical conditions.
- Must have Familiarity with Internet, email systems, and electronic health records (EHRs).
- Bilingual, Spanish and English Speaking (required)
- Must have leadership skills and ability to effectively relay information in writing, telephonically, and with verbal interactions,
