Overview

Adult Outpatient Programs provide the least restrictive level of care where adults may receive clinical interventions aimed at decreasing existing signs and symptoms. Outpatient treatment focuses on recovery, problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Treatment varies widely and depends upon the consumer's presenting symptoms.

Responsibilities

Benefits Specialists are responsible for working with the different benefits providers in the community to ensure that consumers are able to apply for and enroll in their respective resources. They work closely with physicians and represent AltaPointe to advocate on behalf of the consumers.

Essential Functions
  • Assesses consumers' financial resources to determine the need to apply for benefits.
  • Assists consumers in completing applications to secure benefits.
  • Assists consumers in obtaining documentation required for applications to Social Security Administration.
  • Advocates on behalf of consumers with the Social Security Administration office.
  • Effectively utilizes the SOAR system.
  • Tracks consumers' statuses that have applied for benefits.
  • Ensures the Finance and Accounting department and treatment team is notified when benefits are obtained.
  • Ensures matching of appropriate services for multiple consumers in case load.
  • Demonstrates the ability to assess the consumers' needs utilizing the SUN-R.
  • Identifies goals and plans for appropriate services.
  • Provides appropriate linkage to community resources.
  • Makes 72 hour contact with Bridge Aftercare consumers per supervisory request
  • Provides ongoing monitoring of consumers' progress and needs as assessed in the SUN-R.
  • Advocates on behalf of the consumer to secure community resources.
  • Effectively collaborates with the treatment team members and outside sources in the delivery of case management services.
  • Evaluates the consumer for discharge planning.
  • Effectively demonstrates the application of adult specific competencies.
  • Seeks clinical supervision and consultation as needed.
  • Accepts and employs suggestions for improvement.
  • Actively works to enhance case management skills.
  • Documents in a timely fashion per Alta Pointe policy.
  • Documents using Avatar appropriately.
  • Documents in SOAR system in a timely manner.
  • Documents appropriate, complete, and comprehensive SUN-R assessments.
  • Documents measurable and specific goals in the individualized service plan.
  • Documents in a clear and concise manner types of consumer problems, ISP goals addressed and services provided.
  • Documents case disposition, case closings, follow-up and referrals.
  • Documents legibly.
  • Effectively manages caseload based on consumers' needs.
  • Meets AltaPointe productivity standards.
  • Treats consumers with care, dignity and compassion.
  • Respects consumers' privacy and confidentiality.
  • Is pleasant and cooperative with others.
  • Assists consumers and visitors as needed.
  • Personal values don't inhibit ability to relate and care for others.
  • Is sensitive to the consumers' needs, expectations and individual differences.
  • Is gentle and calm to consumers and families.
  • Actively participates in Performance Improvement activities.
  • Actively participates in AltaPointe committees as required.
  • Completes assigned tasks in a timely manner.
  • Follows AltaPointe policies and procedures.
  • Attends required in-service trainings and other workshops.
Qualifications

Minimum Qualifications:

Bachelor degree in a mental health discipline; certification of case management training by Alabama State Department of Mental Health and Mental Retardation; ability to stay organized and multi-task a workload with multiple consumers; knowledge in adult specialty, outlined below, to be attained within six months of employment:
  • Knowledge of psychiatric disorders in the adult population
  • Knowledge of cultural diversity
  • Knowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practice
  • Knowledge of psychiatric rehabilitation principles
  • Knowledge of available community resources
  • Knowledge of educational methods appropriate to adults with severe and persistent psychiatric disorders
  • Knowledge of the social security system in order to obtain benefits
  • Knowledge of the SOAR system
