The Veterans Specialist Care Coordinators works collaboratively with the CCBHC team to coordinate services for consumers who are shared throughout programs. Care Coordinators ensure an inter-grated approach is taken for all consumers volunteering to participate in the CCBHC program.  Care Coordinators will work with primary programs to ensure engagement in treatment programs to meet outlined goals. Care Coordinators will provide nursing support/care coordination to optimize consumer outcomes and improve consumer’s quality of life through clinical support services.

Bachelor's degree in an appropriate mental health discipline

Two or more years’ lived experience with military service or providing support to active-duty members/veterans of the armed forces. Must possess excellent computer and interpersonal skills, punctuality, ability to organize and manage several projects/tasks simultaneously and work independently.  Preferred candidate will have experience working with individuals with mental health and/or substance use disorders.
