SCOPE OF ROLE

Reporting to the Director, Psychiatry Services the RN, Intake will be responsible for the provision of care management and coordinating services including both medical/behavioral health and social service needs. They will also report to the Director, Psychiatry Services for any clinical nursing needs or support. The RN, Intake will be successful by providing accurate and outstanding coordination of all initial services and referrals. They will be relied on to provide nursing support to intake patients and designated intake prescriber.

HOW YOU’LL SUCCEED :

  • Be successful in your role by providing nursing support to intake patients and designated intake prescriber.
  • Create an outstanding patient care experience by validating the patients attended appointments, outcome of the visits, any follow up needed.
  • Get your team up and running efficiently by working in collaboration with internal providers to obtain immediate appointments for those patients at high risk.
  • Facilitate medical records requests, releases of information, and consents to care.
  • Be successful in your role by properly conducting integrated care screenings individually or in group setting.
  • Facilitate and manage care transitions (from intake to internal specialty services) to prevent unnecessary inpatient admissions, inappropriate emergency department use and other adverse outcomes such as homelessness.
  • Work with onsite clinical and administration users to ensure a positive patient experience.
  • Be someone the company can rely on to create a professional and friendly environment at Signature Health when collaborating with patients, families, teams of health care professionals, providers and referral sources.
  • Efficiently conduct or facilitate clinical hand-off interactions between providers to exchange information and ask questions.
  • Track tests and referrals for health care services and follow-up as necessary. Screen out and redirect referrals as needed for best clinical outcomes.
  • Take your team to new heights and develop patient specific self-management plan anticipating possible occurrence or re-occurrences of situations requiring an unscheduled visit or emergency assistance in a crisis.
  • Be someone the company can rely on to provide crisis and triage management for new and existing patients.
  • Comply with all agency policies and procedures.
  • Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.
  • May be required to perform telehealth services as determined by Signature Health.
  • Adherence and completion of compliance training provided by Signature Health.
  • Responsible for and completes all productivity/documentation requirements.
  • Participate in all assigned staff meetings and staff development programs.
  • Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.
  • Other duties as assigned.

KNOWLEDGE & EXPERIENCE :

  • Valid unencumbered RN License in the state of Ohio is required.
  • 3+ years’ experience in as a RN in a Behavioral Health setting is preferred.
  • Extensive experience working with the SPMI population.
  • Must have formal training as well as practical experience in behavioral health and possess core and specialty competencies and skills working with the SPMI population.
  • Must demonstrate either formal training or a strong knowledge base in chronic physical health issues and physical health needs of the SPMI population.

WORKING CONDITIONS :

  • Work is normally performed in a typical interior/office/clinical work environment.
  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
  • Possible exposure to blood borne pathogens while performing job duties.
  • Requires periods of sitting, standing, telephone, and computer work. 
  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
  • Possible exposure to blood borne pathogens while performing job duties.
  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
  • Sufficient dexterity to operate a PC and other office equipment.
