POSITION SUMMARY:

Develop and demonstrate knowledge of the NYS Health Home goals and criteria along with the St. Joseph's Care Coordination care management model. Responsible for outreach activities (mailings, phone calls and face to face visits). Engage and potentially enroll clients on-site as needed. Complete health and psychosocial assessments as needed. Support health home management and staff in development of health home marketing and outreach materials and strategies.

EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION AND LICENSURE:

Associate's degree in related field or commensurate related experience. Marketing or Human Services experience along with strong interpersonal skills

SPECIAL EQUIPMENT, SKILLS OR OTHER REQUIREMENTS:

Must possess strong communication and interpersonal skills. Proficiency in Word, Excel, report writing and power point. Valid Driver's license. Current Defensive Driving Certificate

WORK ENVIRONMENT AND HAZARDS:

Office Setting: Exposure Class II. Limited potential for situations that would cause exposure to body fluids or physical harm

PHYSICAL DEMANDS:

Sedentary work-standing, walking and sitting for prolonged periods of time
