Job Summary:

In this position, the Palliative Care Social Worker provides comprehensive psycho-social assessments, management and support to patients and families at Upstate University Hospitals’ Downtown and Community Campus’. Primary assignment will be with the Palliative Care Team with additional duties as assigned. The Palliative Care Social Worker works closely with the multidisciplinary team and provides insight regarding patients’ quality of life, assisting with decision making determination and advance directives. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. In addition, the social worker will actively manage patients through collaboration, coordination, monitoring, evaluating options and identifying required services to meet individual's needs. Candidate must possess communication skills, have knowledge of social determinants, population health and available community resources; document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.

Minimum Qualifications:

M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Preferred Qualifications:

Post Master's work experience.

Work Days: Monday-Friday, 8:30 am - 5:00 pm, with occasional on call/holiday coverage on a rotating basis to meet operational needs
