The Social Worker works collaboratively with the Care Managers and other members of the multidisciplinary healthcare team to ensure the appropriate allocation of hospital resources while maintaining the quality of care.   The Social Worker participates in the development and implementation of a comprehensive integrated discharge plan.   The Social Worker identifies patient and family psychosocial and environmental needs to admission, diagnosis, treatment and discharge.  The Social Worker actively participates in the discharge process following federal and state regulations and accreditation standards managing the care across the continuum to ensure a safe discharge within a timely manner.  The Social Worker provides crisis interventions to patients and families as needed.


Job Requirements:

  • MINIMUM EDUCATION / CERTIFICATION / LICENSURE
    • Required: Masters Degree in Social Work.
    • Preferred: LMSW licensing preferred
  • EXPERIENCE
    • Required: 2 - 3 years Social Work experience.
    • Preferred: Case Management, Discharge Planning or home care experience preferred.
  • SPECIAL SKILLS/EQUIPMENT
    • Excellent oral and written communication and interpersonal skills. Demonstrate age appropriate communication in assigned work area. Ability to interact with physicians, members of the multidisciplinary healthcare team, patients and families and community agencies to achieve optimal patient outcomes. Knowledge of federal and state regulations (DOH, Medicare/Medicare). Strong organizational and problem solving skills. Knowledge of third party payers and /or managed care principles.

Work Type: Full-Time
Work Hours: 8:00am-4:30pm w/ weekend coverage (1-2 shifts per month)
