Diabetes Educator

Opening Family Practice is seeking a Diabetes Educator to assist in our Patient-Centered Medical Home (PCMH) model of care as part of the Comprehensive Primary Care Plus initiative (CPC+) Requires Registered Nurse or Registered Dietician with a minimum of a Bachelor's degree, and CO licensure.

Experience in primary care and diabetes disease expertise is desired. Certified Diabetes Educator Preferred.

This individual would work with patients and individuals in the community in deference to age and cultural diversity. Educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition
