  • Initiates and maintains a high level of customer service and positive employee relationships.
  • Evaluates and processes homecare referrals appropriately.
  • Demonstrates knowledge and ability to appropriately pre-certify homecare patients with managed care contracts.
  • Establishes and promotes an ongoing communication regarding referrals and pre-certification with appropriate parties.
  • Provides support to UHHCS pharmacy services.
  • Knowledge of Medicare and Medicaid regulations and reimbursement.
  • This is the day shift.
  • Monday-Friday with rotating weekends and holidays.
  • The shift is PRN

  • RN - Registered Nurse Graduate of an approved School of Nursing.
  • BSN preferred.
  • One year of home health care, hospital discharge planning and/or case management preferred.
  • Two years experience in the acute care setting.
