Job Summary: In this position, the Social Worker will provide comprehensive psycho-social assessment, management and support to at risk patients and families in the inpatient and outpatient pediatric hematology and oncology population. In addition, the social worker addresses social determinants and complex issues.  Will assist and collaborate to make referrals with multiple community agencies to meet the needs of the patients and families. The social worker works closely with the multidisciplinary team. This position is part of the interdisciplinary team that recommends and delivers appropriate social work intervention and support. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.   Minimum Qualifications:

MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required.

**New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

  Preferred Qualifications:

 

  Work Days: Monday - Friday 8:30am - 5:00pm with occasional on call/holiday coverage on a rotating basis to meet operational needs.              Salary Range/Pay Rate:
