Essential Functions/Responsibilities/Duties

  • Provides care during patient visits, discussing chief complaint and obtaining vital signs and complete health history for the patient. Develops a baseline physical assessment.
  • Performs phone triage with patients to determine chief complaint and assessment of next steps and follow up care needed.
  • Documents health history, assessments, actions and medications in the Electronic Medical Record (EMR) system.
  • Assists providers in diagnostic procedures and treatments.
  • Performs IV Starts when ordered. Provides information to patient regarding the procedure, including site selection and possible complications.
  • Communicates with patient to collect information on allergies and preexisting conditions.
  • Provides information to patient regarding the procedure, including site selection and possible complications.
  • Identifies the purpose, side effects, and onset of action for each class of drug being given to patient. Explains medication, usage and possible side effects to patient/guardian and obtains consent.
  • Collaborates with other healthcare providers in developing patient plan of care. Coordinates and plans interagency referrals related to patient care as directed by the provider.
  • Collaborates with provider in implementing the patient plan of care including providing education and identifying additional services needed for the patient. Initiates follow-up care plan as needed.
  • Collaborates with ancillary staff to process patients based on providers’ schedule, maintaining smooth patient flow.
  • Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.

Qualifications

  • Education:Associate’s Degree in Nursing.Bachelor of Science Degree in Nursing preferred.
  • Experience:One year’s experience providing clinical or outpatient care as an Registered Nurse (RN) preferred.
  • Professional Licenses/Certificates/Registration:RN license for the state of practice.Cardiopulmonary Resuscitation (CPR) certification within three months of employment.BCLS (Basic Cardiac Life Support) certification.ACLS (Advanced Cardiac Life Support) preferred.PALS (Pediatric Advanced Life Support) preferred.
  • Knowledge/Skills/Abilities:Bilingual (English/Spanish) preferred. New Hope is a nurse-managed program that is part of Yakima Valley Farm Workers Clinic. New Hope specializes in caring for HIV patients in Central Washington, offering medical, dental, nutrition, counseling, outreach, and mental health services. New Hope also trains healthcare providers on caring for HIV patients and is funded by a Ryan White Part C grant.

We’ve transformed into a leading community health center. With 40+ clinics across Washington and Oregon, we offer a wide range of services such as medical, dental, pharmacy, orthodontia, nutritional counseling, autism screening, and behavioral health. Our holistic model also assists with shelter, energy, weatherization, home visits, and mobile medical/dental clinics.

As a Public Health Nurse, you will have the opportunity to:

  • Provide direct nursing care and support to patients and medical providers, including wellness activities like immunizations and screenings.

  • Collaborate with patients and healthcare providers to develop care plans based on medical conditions, goals, and behaviors. Coordinate referrals as directed.

  • Assess and educate patients on readiness for Antiretroviral Therapy (ART) and manage those on HAART therapy.

  • Administer medication and explain its purpose, side effects, and onset of action.

  • Conduct home visits for assessment, follow-up, education, and medication monitoring.

  • Continuously assess, intervene, educate, and refer patients as per the care plan.

  • Maintain regular contact with other services and professionals for coordinated care.

  • Offer HIV counseling and testing to high-risk individuals.

  • Complete chart notes, update EMR data, and document assessments, care plans, actions, and medications.

  • Educate various audiences on HIV transmission and risk reduction.

  • Follow The Joint Commission, HRSA, and YVFWC requirements.

  • Perform additional assigned duties as needed.

Qualifications:

  • Education: An Associate’s Degree in Nursing. Bachelor’s Degree in Nursing preferred. 

  • Experience: Experience working with HIV/AIDS preferred. Training and six months’ experience providing adult physical exams is preferred. 

  • Professional Licenses/Certificates/Registration: WA state RN license. CPR certification within 90 days of employment. BLS Certification is required. ACLS Certification preferred. Valid Driver’s License and proof of automobile liability insurance coverage.

  • Knowledge/Skills/Abilities:

Position Highlights:

  • Clinic Hours: Monday – Friday, 8:00 am-7:00 pm
  • May require Saturday and Holiday shifts
  • Floats to other sites within the region as needed
  • 8hr and 10hr shifts available

Essential Functions/Responsibilities/Duties:

  • Provide care during patient visits, discuss chief complaints, obtain vital signs, and complete health history for the patient; develop a baseline physical assessment.
  • Perform phone triage with patients to determine chief complaint and assess next steps and follow-up care needed.
  • Document health history, assessments, actions, and medications in the Electronic Medical Record (EMR) system.
  • Assist providers in diagnostic procedures and treatments.
  • Perform IV Starts when ordered. Provides patient information regarding the procedure, including site selection and possible complications.
  • Communicate with patients to collect information on allergies and preexisting conditions.
  • Provide information to the patient regarding the procedure, including site selection and possible complications.
  • Identify the purpose, side effects, and onset of action for each drug class given to the patient. Explain medication, usage, and possible side effects to the patient/guardian and obtain consent.
  • Collaborate with other healthcare providers in developing patient plans of care. Coordinates and plans interagency referrals for patient care as directed by the provider.
  • Collaborate with providers in implementing the patient plan of care, including providing education and identifying additional services needed for the patient. Initiates follow-up care plan as needed.
  • Collaborate with ancillary staff to process patients based on providers’ schedules, maintaining smooth patient flow.
  • Perform clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.
  • Perform other duties as assigned.

Qualifications:

  • Education:Associate’s Degree in Nursing required, Bachelor of Science Degree in Nursing welcomed.
  • Professional Licenses/Certificates/Registration:RN License for the state of practice required, CPR certification within three months of employment required, BCLS certification (renewed annually), ACLS welcomed, PALS welcomed.
  • Knowledge/Skills/Abilities:Bilingual (English/Spanish) welcomed; must demonstrate level 10 proficiency for bilingual differential pay, Training in Early Periodic Screening & Development Testing and Women’s Health Exams preferred, Proficient knowledge in nursing concepts, procedures, and medication uses and effects, Effective verbal, written, and listening communication skills, Ability to prioritize tasks and handle multiple responsibilities with interruptions, Ability to interact with patients, families, and employees professionally, Ability to understand and meet the unique needs of patients of all ages, Basic proficiency in Word, Excel, and EMR.

Program Leadership:

Ensure Care Plan Implementation and adherence:

Qualifications

  • Education: Bachelor’s Degree in Nursing. Associates Degree and Two years’ additional directly related experience may be substituted for the required education.  HS Diploma and Four years’ additional directly related experience may be substituted for the required education.  
  • Experience: Five years’ experience in care coordination, case/care management, utilization/resource management, quality management, and/or transition of care. This includes two years’ experience supervising staff. 
  • Professional Licenses/Certificates/Registration: Registered Nurse (RN) with current licensure to practice in Washington State. Case Management Certification (CCM) is preferred. 
  • Knowledge/Skills/Abilities: Bilingual (English/Spanish) preferred. This position is not eligible for bilingual differential pay. Knowledge of utilization management, quality improvement, discharge planning, and cost management. Knowledge of the uses and effects of medications. Knowledge of cultural differences and ability to work effectively with culturally diverse populations. Effective verbal, written and listening communication skills. As a Public Health Nurse, you will have the opportunity to:

  • Provide comprehensive specialized care and support to patients living with HIV/AIDS by monitoring and assessing patients’ health, administering medications, and managing symptoms related to HIV/AIDS. 
  • Provide emotional support and counseling to patients, addressing their fears, concerns, and challenges related to living with HIV/AIDS.
  • Maintain wellness activities for patients, including immunizations, screening exams, and follow-up lab work. Offers guidance on safe practices, including safe sex and needle exchange programs comma, to prevent the spread of HIV/AIDS.
  • Develop care plans with patients in collaboration with providers based on medical conditions, health goals, and behaviors. Collaborates with other healthcare providers in developing a plan of care. Coordinates and plans interagency referrals for patient care as directed by the provider.
  • Provide assessment, education, and evaluation of patients’ readiness to begin Antiretroviral Therapy (ART). Provides ongoing long-term management for patients receiving HAART therapy.
  • Administer medication to patients as directed by the provider, protocol, and standing orders. Identifies the purpose, side effects, and onset of action for each drug class given to the patient. Correctly explains medication to patient/guardian and obtains consent. Regularly monitors patients’ viral loads, CD4 counts, and overall health to track progress and adjust treatment plans as necessary.
  • Follow strict infection control protocols to prevent the transmission of HIV/AIDs within the clinic setting.
  • Maintain appropriate ongoing assessment, interventions, education, and referrals as identified in the care plan.
  • Maintain regular contact and work collaboratively with other services and professionals involved in delivering services to clients to ensure coordinated service delivery.
  • Provide HIV counseling and coordination of testing for high-risk individuals.
  • Complete chart notes per standards in EPIC and Provide systems. Updates client data in the Electronic Medical Record (EMR) systems to document health assessment and health history. Records care plans and education provided. Documents assessments, actions, and medications in medical records.
  • Provide education to a wide variety of audiences, including community members and healthcare professionals, regarding HIV-including transmission and risk reduction.
  • Community outreach programs to raise awareness about HIV/AIDS, promote testing, and provide support to affected individuals and their families.
  • Advocate for patients’ rights and access to health care services, ensuring they receive appropriate medical and social support
  • Crisis management: effectively handles medical emergencies and crises, providing immediate care and coordinating with the health care team. This includes taking calls for after hours.
  • Continuing education: stay updated with the latest advancements in HIV/AIDS treatments, medications, and healthcare practices through continued education and training
  • Perform other duties as assigned.
  • Perform clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), Department of Health (DOH), and YVFWC requirements.

Qualifications:

  • Education: An Associate’s Degree in Nursing is required. Bachelor’s Degree in Nursing preferred. 
  • Professional Licenses/Certificates/Registration: WA state RN license. CPR certification within 90 days of employment. BLS Certification is required. ACLS Certification preferred. Valid Driver’s License and proof of automobile liability insurance coverage.

Essential Functions/Responsibilities/Duties

The Nurse-Family Partnership (NFP) Nurse Home Visitor (NHV) is responsible for providing comprehensive nursing services to women and their families through the child’s 2nd birthday. NHVs are pivotal in transforming the lives of babies, mothers and families by implementing and maintaining the fidelity of the NFP model. Our goal is that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken. NHVs report to the NFP Nurse Supervisor.   • Provides home visits to first time moms and their families through the child’s 2nd birthday.
  • Adheres to nursing process and the NFP model of home visitation.

Working Conditions

  • Must be able to work a flexible schedule including some evening and weekends to meet the needs of the families we serve.

Qualifications

  • Experience: Six months’ experience providing newborn exams.

As an RN Charge Supervisor, you will have the opportunity to:

  • Coach, train, and evaluate staff performance
  • Facilitate growth through training plans
  • Participate in recruitment and orientation of new staff
  • Manage department workflow and patient schedules
  • Conduct patient assessments and educate on procedures and medications
  • Collaborate on care plans and referrals
  • Document in EMR systems
  • Support providers in procedures and treatments
  • Oversee medication administration and patient education
  • Plan meetings and monitor program progress
  • Ensure compliance with care processes and engage staff in improvement
  • Liaise with insurance companies and managed care plans along with other assigned duties

Minimum Qualifications:

  • Associate’s Degree in Nursing
  • One year’s experience as a Registered Nurse (RN)
  • Two years of supervisory experience is preferred
  • WA State RN license
  • BCLS certification is required within 90 days of hire
  • Proficient in nursing principles and procedures & familiar with medication use and effects
  • Strong communication, problem-solving, and conflict-resolution skills
  • Capable of multitasking and prioritizing effectively
  • Skilled in professional interactions with patients, families, and colleagues & adept at addressing diverse patient needs

Who we are looking for:

An experienced RN Care Manager to work with patients to improve their health through coaching, referral, and connection to services and education!

As a Care Management Nurse, your tasks will include:

  • Writing and documenting care plans, motivational interviewing, and building relationships with patients
  • Gathering detailed patient information to identify needs, set goals, and collaborate with the care team to create individualized care plans
  • Implementing care activities, maintaining regular contact with patients, and tracking plan effectiveness
  • Utilizing clinical expertise to ensure compliance with medical guidelines and provide patient education
  • Advocating for patients, build relationships with service providers, and provide feedback on care programs
  • Participating in meetings, provide training, maintain patient files, document program activities, and adhere to healthcare regulations

Minimum Qualifications:

Education: Bachelor’s Degree in Nursing or an Associate’s Degree and two years of directly related RN experience

Experience

  • Two years of patient-centered care
  • Experience with complex patients with advanced, chronic, and behavioral health illnesses and multiple co-morbidities in an ambulatory setting
  • Diverse clinical experience in working with Medicaid, Medicare, and private insurance beneficiaries with two or more chronic conditions

Certifications and Licenses

  • Chronic Care Professional (CCP) Certification or the Iowa Chronic Care Consortium Certification is preferred
  • Washington State RN license
  • If an RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply:
    • Must submit proof of license in state of residence
    • Must submit proof of completion for the required suicide prevention training during 1st week of onboarding
    • If relocating to Washington State, must obtain WA RN license within 6 months of hire
    • Must update primary state of residence within 60 days (applies to anyone relocating to Washington State)
    • Military Nurses are not required to obtain a Nurse Licensure Compact (NLC)
  • Must obtain Oregon State RN license within 180 days of hire

Skills

As a Clinical Nurse Supervisor, you will have the opportunity to:

  • Coaches, mentors and trains direct report staff. Assesses competencies of staff, provides continuous performance management and conducts employee performance evaluations. Provides feedback and progress related to site and individual goals and objectives. Addresses performance and/or behavior issues, clarifying expectations and providing feedback.
  • Supports staff growth and development. Assesses the educational, clinical experience, and communication needs of staff in collaboration with the individual. Encourages staff to seek educational opportunities incorporating job related training in addition to mandatory training.
  • Collaborates with Nursing and Medical Assistant staff to create and implement ideas for education including clinical performance and effective communication.
  • Participates in the recruitment and selection of staff. Provides orientation to outline job requirements and expectations, policies and procedures, and proper use of tools and equipment.
  • Develops and implements annual department work plan forecasting equipment needs and capacity requirements.
  • Delegates, assigns and coordinates work, ensuring an efficient work flow that achieves the departmental mission and objectives.
  • Monitors staff compliance with all policies, procedures and standards of Yakima Valley Farm Workers Clinic (YVFWC). Ensures clinic compliance with Health Insurance Portability and Accountability Act (HIPPA), The Joint Commission (TJC), and Occupational Safety & Health Administration (OSHA) standards of patient and employee safety.
  • Collaborates with management in the clinic as well as other clinics to plan patient care delivery systems to effectively address patient needs and ensure quality patient care.
  • Leads the Nursing and Medical Assistant staff in the daily implementation of patient care plans and workflows that meet state and federal requirements and/or metrics.
  • Reviews processes related to daily implementation of patient care and assignments. Evaluates adherence to process and works with staff on process improvement solutions.
  • Collaborates with clinic and maintenance staff to maintain the facility and ensure equipment and supplies are in good working order.
  • Performs other duties as assigned.
  • Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.

Qualifications

  • Education: Associate’s Degree in Nursing. Bachelor of Science Degree in Nursing preferred.
  • Experience: Two years’ experience in nursing or healthcare operations. One year in Nursing Supervisory capacity preferred.
  • Licenses/Certificates/Registration:Registered Nurse license for the state of practice. Cardiopulmonary Resuscitation (CPR) certification within 90 days of hire. Advanced Cardiac Life Support (ACLS) preferred. Pediatric Advanced Life Support (PALS) preferred.
  • Knowledge/Skills/Abilities Required: Bilingual (English/Spanish) preferred. Must demonstrate the ability to communicate at level 10 on the language proficiency scale to receive bilingual differential pay. Knowledge of established professional nursing concepts, principles of practice and nursing procedures. Ability to determine, assess and react effectively to the medical needs of patients in a clinic. Knowledge of the uses and effects of medications. Effective verbal, written and listening communication skills. Skills in problem-solving, sound judgement and conflict resolution.   • Conduct phone triage to assess patient needs and coordinate follow-up care
  • Assist with diagnostic procedures and treatments, including IV starts when ordered, while educating patients on procedures and medications
  • Collaborate with the healthcare team to develop and implement patient care plans, coordinate referrals, and provide education to patients on medications and potential side effects

Qualifications:

  • Associate’s Degree in Nursing
  • One year’s experience providing clinical or outpatient care as an RN is preferred
  • WA State RN license
  • If an RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply: Must submit proof of license in state of residence. Must submit proof of completion for the required suicide prevention training during 1st week of onboarding. If relocating to Washington State, must obtain WA RN license within 6 months of hire. Must update the primary state of residence within 60 days (applies to anyone relocating to Washington State). Military Nurses are not required to obtain a Nurse Licensure Compact (NLC)

As a Registered Nurse, you will have the opportunity to:

  • Provides care during patient visits, discussing chief complaint and obtaining vital signs and complete health history for the patient. Develops a baseline physical assessment.
  • Performs phone triage with patients to determine chief complaint and assessment of next steps and follow up care needed.
  • Documents health history, assessments, actions and medications in the Electronic Medical Record (EMR) system.
  • Assists providers in diagnostic procedures and treatments.
  • Performs IV Starts when ordered. Provides information to patient regarding the procedure, including site selection and possible complications.
  • Communicates with patient to collect information on allergies and preexisting conditions.
  • Provides information to patient regarding the procedure, including site selection and possible complications.
  • Identifies the purpose, side effects, and onset of action for each class of drug being given to patient. Explains medication, usage and possible side effects to patient/guardian and obtains consent.
  • Collaborates with other healthcare providers in developing patient plan of care. Coordinates and plans interagency referrals related to patient care as directed by the provider.
  • Collaborates with provider in implementing the patient plan of care including providing education and identifying additional services needed for the patient. Initiates follow-up care plan as needed.
  • Collaborates with ancillary staff to process patients based on providers’ schedule, maintaining smooth patient flow.
  • Performs other duties as assigned.

As an RN Charge Supervisor, you will have the opportunity to:

  • Coach, train, and evaluate staff performance
  • Facilitate growth through training plans
  • Participate in recruitment and orientation of new staff
  • Manage department workflow and patient schedules
  • Conduct patient assessments and educate on procedures and medications
  • Collaborate on care plans and referrals
  • Document in EMR systems
  • Support providers in procedures and treatments
  • Oversee medication administration and patient education
  • Plan meetings and monitor program progress
  • Ensure compliance with care processes and engage staff in improvement
  • Liaise with insurance companies and managed care plans along with other assigned duties

Minimum Qualifications:

  • Associate’s Degree in Nursing
  • One year’s experience as a Registered Nurse (RN)
  • Two years of supervisory experience is preferred
  • WA State RN license
  • BCLS certification is required within 90 days of hire
  • Bilingual (English/Spanish) is preferred at a level 10 to receive bilingual differential pay
  • Proficient in nursing principles and procedures & familiar with medication use and effects
  • Strong communication, problem-solving, and conflict-resolution skills
  • Capable of multitasking and prioritizing effectively

Position Highlights:

Qualifications:

  • Education:Associate’s Degree in Nursing.
  • Experience:One year’s experience providing clinical or outpatient care as an RN is preferred
  • Licenses & Certifications: State RN license. If an RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply:
    • Must submit proof of license in state of residence.
    • Must submit proof of completion for the required suicide prevention training during 1st week of onboarding.
    • If relocating to Washington State, must obtain WA RN license within 6 months of hire.
    • Must update primary state of residence within 60 days (applies to anyone relocating to Washington State)
    • Military Nurses are not required to obtain a Nurse Licensure Compact (NLC)
  • BCLS certification within 90 days of hire; ACLS & PALS are preferred.
