Yamhill County has one regular, full-time position in the Health & Human Services Community Support Services Division for an intensive case-manager (HS Specialist I) to provide person-centered services and supports serving individuals with serious mental health or co-occurring mental health and substance use conditions as part of a transdisciplinary Assertive Community Treatment (ACT) team. ACT services are largely community-based. Primary duties and responsibilities include, intensive case management, collaborative service plan development, skills training, and providing linkages to medical care or other resources and benefits. Additional duties may include providing support and data tracking. Candidates must embrace the philosophy of resilience and recovery. The Qualifications:

The position requires a bachelor’s degree in a related field in addition to at least one year of experience in a setting relevant to the duties and skills noted above. An appropriate combination of education and experience may be substituted for degree. May need to meet or have the ability to meet, he qualifications of CADC to work in chemical dependency program and/or meet the QMHA requirement in the mental health program within one year of the date of hire. A valid license to drive in the state of Oregon is required in addition to a good driving record. Must pass a criminal background check.
 

Special Requirements:

The State of Oregon requires all Health Services and Clinic employees to complete their COVID-19 vaccination series or have an approved exemption to the requirement due to a medical condition or sincerely held religious belief. Successful candidates for this position must submit vaccination documentation or be approved for an exemption prior to their first day of employment. Failure to provide proof of full documentation or receipt of an approved exemption will lead to withdrawal of the job offer.

 
The Candidate:

The successful candidate will have working knowledge of techniques specifically related to community mental health services and the following abilities: to understand the behavior of people under stress; to develop and maintain effective cooperative relationships with clients and their families, the community, physicians, law enforcement agencies, the courts, and public and private administrators; to interpret community mental health and chemical dependency services, and to prepare concise and complete client treatment and progress records. Typing and computer proficiency is required for collaborative documentation. Additionally, they will have excellent communication skills, and will be dependable, organized, self-directed, detail-oriented, and possess the ability to work in a team environment and to interact with co-workers, clients, and the public in a courteous, professional manner.
