Coordinator, Northridge Commons

 

SCOPE OF ROLE

Reporting to the Manager, BH Services, the Coordinator, Northridge Commons is responsible for coordinating daily operation of the group home and its services for the Northridge Commons Supportive Housing Project residents. You will schedule, train, and coordinate daily activities of peer support employees. The Coordinator, Northridge Commons acts as a case manager, advocate and resource for residents.

HOW YOU’LL SUCCEED ____________________________________________________________________

  • Schedule, train, and coordinate daily operations of peer support employees.
  • Develop and maintain all necessary records and reports as directed by the Manager, BH Services.
  • Develop and administer house policies and operating procedures.
  • Protect the rights of residents and operate as an advocate and resource for such residents.
  • Responsible for regulatory compliance requirements, including adherence to ODMH licensing standards.
  • Consult regularly with residents and Signature Health employees to discuss resident, facility program concerns and to facilitate the residents’ ISP.
  • Serve as a liaison between Signature Health and the landlord Eden, Inc., to ensure facility meets established safety standards.
  • Assist SMD/SED clients to meet educational, medical, vocational, housing and daily living skill needs enabling clients to successfully remain in the community.
  • Maintain client charting in accordance with Medicaid, JCAHO, ODADAS, ODMH, CARF and professional standards.
  • Assist in crisis intervention.
  • Comply with all agency policies and procedures.
  • Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.
  • May be required to perform remote services as determined by Signature Health.
  • Adherence and completion of compliance training provided by Signature Health.
  • Responsible for and completes all productivity/documentation requirements.
  • Participate in all assigned staff meetings and staff development programs.
  • Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.
  • Other duties as assigned. 

KNOWLEDGE & EXPERIENCE ______________________________________________________________

  • High School Diploma or equivalent required.
  • 5+ years' of previous case management experience required.
  • 3-5+ years’ experience in a residential setting for the elderly and/or adults with mental illness preferred.

  • American Heart Association (AHA) Basic Life Support (BLS) certification required.

  •  Bachelor’s Degree in social services field preferred.

WORKING CONDITIONS ___________________________________________________________________

  • Work is normally performed in a typical interior/office/clinical work environment.
  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
  • Requires periods of sitting, standing, telephone, and computer work. 
  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
  • Possible exposure to blood borne pathogens while performing job duties.
  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
  • Sufficient dexterity to operate a PC and other office equipment.
