Job Summary: This pharmacist will operate under our specialty pharmacy services umbrella, and will primarily be responsible for providing clinical pharmacy services to the Upstate Medical University Cancer Center Clinic. This pharmacist will work closely with patients, providers, and nurses within the Upstate Cancer Center to optimize patient care. They will assess appropriateness of new oral chemotherapy regimens and provide patient specific education prior to initiation and first dispensing. They will also coordinate care with the Upstate Specialty Pharmacies (The SUNY Outpatient Pharmacies) for medication counseling, adherence monitoring, and therapy acquisition. Clinical activities will include, but are not limited to, direct patient care and medication oversight, patient counseling, formulary management, provider/staff/resident/student education, specialty pharmaceutical coordination, and transitions of care coordination. This individual will join a team of specialty clinical pharmacists, providers, and nurses to provide coordinated patient care. As a member of the outpatient pharmacy department, all pharmacists should also be able to perform the following functions as needed by the department: interprets medication orders and prescriptions to ensure that they are complete, safe, rational, and compatible with institutional policy. Performs duties in all areas of Pharmacy to include outpatient dispensing pharmacy, central telephone education outreach and processing medication orders. This relates to clinical activities, metric reporting for compliance, and quality improvement projects for specialty pharmacy accreditation. This pharmacist will oversee program expansion for specialty oral oncology medication services, focus on developing relationships with Upstate Cancer Center clinic staff and specialty providers, help build third party payer and manufacturer relations, and participate in all areas of dispensing, call center and clinic as needed or requested. Provides information to health care practitioners to promote the rational use of medications. Provides drug monitoring service. Maintains Pharmacy records by law and institutional policy. Interprets policy and procedures for hospital staff. Participates in teaching programs for hospital staff. Participates in Specialty Pharmacy Quality Management Committee.
Minimum Qualifications:

B.S. in Pharmacy or Pharm.D., NYS license and registration - or eligible - required; completion of PGY1 Pharmacy residency or equivalent experience required.

Preferred Qualifications:

Previous inpatient or outpatient hematology/oncology pharmacy and/or ambulatory care practice experience preferred.. PGY-2 residency trained and/or Board Certified Oncology Pharmacists (BCOP) preferred.

Work Days:

Day shifts, Monday-Friday. Hours dependent on the operating needs of the Outpatient Pharmacy and Cancer Center. Possibility of weekends or evening based on department need.
