Summary

THE POSITION 

Public Health - Seattle and King County is looking for a dedicated part time Social Worker for our Downtown Public Health Center. This Social Worker will work as part of a team in a fast-paced Public Health Clinic offering Primary care services, Dental services, Services for to low-income families, and Refugees, many with medical complexity and social vulnerability, specifically homelessness.  We value low-barrier access to high quality care for all patients and strive to reduce institutional and systemic barriers to that care.

The Social Worker provides services building-wide, including crisis intervention, case management and care coordination and referral to resources to meet the patient’s needs. Performance of duties may be autonomous and self-directed, or provided as part of a multi-disciplinary team.

This is a TLT (temporary) position, the assignment ends 12/31/23.  This recruitment will remain open until filled.

Job Duties

EXAMPLES OF DUTIES 

  • Provide Social Work support to access behavioral health programs and assist in navigation and successful linkage to community-based programs.  Services can include assessment of needs, review of programs the client is currently enrolled in and assistance in re-engagement with that program, review of eligibility for new programs and referral. 
  • Provide referral to other resources that address social determinants of health and that will improve health outcomes. 
  • Provide mental health crisis intervention and coordination with community crisis services for individuals seeking services at the Downtown Public Health Center.  This will include those who are current Public Health clients and those who may not be a Public Health client but come to the site.
  • Assist site management and staff with responding to clients or others that require de-escalation, behavioral health support, mental health first aid, linkage to resources or other support to increase quality of care for those seeking services and to maximize safety for clients and staff. 
  • Work in a complementary manner with security personnel, when necessary, as part of a collective response to incidents when a client is in need of assistance with behavioral health intervention, de-escalation, etc.
  • Document referrals and interventions in EHR and other required systems. 
  • Work collaboratively as part of a team with other disciplines in Public Health and the community. 
  • Provide support and education for other Public Health staff on crisis response, de-escalation skills, and reduction of secondary trauma. 
  • Coordinate with supervisor regarding activity priorities according to program and site needs.

Experience, Qualifications, Knowledge, Skills

QUALIFICATIONS 

  • Master's degree in Social Work, Marriage and Family Therapy, or Counseling/Behavioral Health from an accredited university. 
  • At minimum, be able to obtain an agency affiliated DOH credential. 
  • Experience with assessment and treatment planning for common mental health and/or substance use disorders.  
  • Experience and skills in crisis intervention, de-escalation, and crisis services in King County. 
  • Ability to effectively engage clients in a therapeutic relationship, and ability to work with clients by telephone as well as in person.  
  • Demonstrated ability to communicate, both orally and in writing, to foster and maintain a variety of interpersonal relationships and to provide social work services independently with minimal immediate supervision and as part of a team.
  • Demonstrated ability to apply professional social work principles, methodology and ethics. 
  • Knowledge of equity and social justice principles and practice and understanding of the effects of health disparities and population health.
  • Demonstrated ability to maintain a high level of organization to meet high volume workload demands. 
  • Working knowledge of standard software applications such as Word, Excel, Outlook, and Teams. 
  • Ability to travel in a timely manner within King County to locations that may have limited access to public transportation.  
  • Demonstrated ability to collaborate and communicate effectively in a team setting.

Any equivalent combination of experience and education which provides the applicant with the desired skills, knowledge and ability required to perform the work.
 
DESIRABLE QUALIFICATIONS: 
  • Bilingual in English and Spanish preferred. 
  • Post graduate professional experience providing independent social work services in a health or social service setting. 
  • Experience providing services to high-risk clients of low income, multi-lingual, multi-ethnic, and multi-cultural populations.

Work location: The location is our Downtown Public Health Center

Work Schedule: This is a part-time position, 0.75 FTE, 30 hours per week job, Monday to Friday, 10 AM – 5 PM, and depending on business and operational needs, work hours may vary at times. It is non-exempt and therefore subject to the provisions of the Fair Labor Standards Act.
