The UWMC Surgery Clinic has an outstanding opportunity for a Program Coordinator (Full-Time, Day Shifts). Working within the Surgical Specialties Center in the sub-specialty of Vascular Surgery, the Program Coordinator demonstrates service excellence standards in independently performing the following support services:

•Work with nurses, physicians, program coordinators, patient services specialists, medical assistants, patient care coordinators and other departmental staff members on various projects.
•Coordinate and schedule extensive diagnostic tests necessary for evaluation and coordinate continuum of care for Vascular Surgery

Job duties include:

Act as an educational, informational resource for program participants, including patients and staff:
•Being knowledgeable of types of studies needed, coordination of care, services provided by the program and resources available to out of town patients.

Participate as a team member in efforts to promote the program:
•Develop communication materials about the program, design/maintain provider templates and create process improvements to enhance the patient experience and improve workflows.

Register new patients and verify insurance coverage:
•Obtain pre-authorization for office consults and diagnostic studies for the motility lab.

Respond to inquiries regarding clinic business operations:
•Answer phones and triage patient calls.
•Follow-up on referring provider and agency requests.
•Schedule for multiple services and insure that all appointments are well-coordinated to minimize patient travel time.
•Mentor and assure staff orientation of new volunteer and student workers in the clinic.
•Demonstrate proficiency with use of multiple business operations computer systems at UWMC including EPIC, Mindscape and Outlook program databases.
•Attend monthly CEGS meetings and other meetings as required.
•Report patient concerns to MD/RN regarding their medical, social, financial, psychological or cultural needs as it relates to the planning, personal comfort, or safety of the planned test or procedure.
•Interact with patient to discuss itinerary of the diagnostic tests, pre-test patient preparation instructions, and answer questions.
•Order interpreter and translation of correspondence as needed.
•Coordinate multidisciplinary consults (Med oncology, Radiation Oncology, Nutritionist, Surgery consult)
•Collaborate with RN II/III to improve and coordinate front to back operations. Consult with RN regarding complicated follow up patient care coordination and schedule complex patients that require additional care.
•Responsibility for continuum of care steps preceding and following initial appointment: requesting and ensuring timely receipt of outside records using HIM; test, procedure and appointment scheduling; outgoing referrals and coordination of care, and completion of missing outside provider documentation.  Contact various departments to schedule program tests; complete necessary requisitions and consult at UWMC and/or outside facilities.
•Prepare schedule for diagnostic tests and clinic, and correspond with patients to provide necessary program instructions.
•Review patient history and determine based on pervious diagnostic test and history what diagnostic test should be scheduled with the motility lab or digestive disease center (such as manometry, ph study, endoscopy, or barium swallow)

Manage files:
•Coordinate the receipt of requirements: patient forms, medical records, lab and diagnostic results from UWMC, primary care physicians’ offices, pulmonologists’ offices and other medical facilities.
•Receive outside laboratory results.
•Ensure results are delivered to MD/RN2 to recommend next phase of care to coordinate.
•In preparation for return clinic visits, ensure recent lab work, radiology results, and pathology reports are completed and available.
•Follow up to ensure the exams are completed or rescheduled if necessary and notify MD/RN of unusual delays in the completion of exams. REQUIREMENTS: High school graduation or equivalent AND two years of experience in the program specialty OR equivalent education/experience.
