Description

JOB SUMMARY:

  • Serves as a single point of contact for referring physicians, patients, and community partners to reduce barriers to needed resources
  • Directing clients to sources of emotional, financial, administrative, or cultural support
  • Build working relationships, solve problems and support patients as they learn to self-navigate the system

QUALIFICATIONS:

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Coordinate services for individuals in need, including substance use disorder treatment, counseling, transportation, financial assistance, nutrition and housing needs
  • Stay informed with changes in the service delivery system and seek current information about available resources
  • Manage and maintain a client data tracking system, including updated information, demographics and referrals made and followed up on
  • Building client relationships and trust by being an effective communicator and listener, while understanding your role's boundaries and maintaining a professional relationship
  • Responsible for networking and outreach efforts to establish and maintain positive working relationships with key community partners
  • Perform navigation work with minimal supervision. May be required to occasionally work irregular hours, attend other job-related meetings and/or training, and perform other duties as assigned

COMMUNICATION RESPONSIBILITIES:

  • Continuously works to enhance general public awareness of the Hancock Health and healthy365 Connection Center and recruit partners as part of an ever-expanding network.
  • Cultivates community and business relationships to ensure a strong local commitment to promote wellness for generations to come.
  • Effectively and consistently communicates with staff and encourage interactive meetings and discussions.

Position Requirements

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Bachelor’s degree in social work, counseling, or a related field required
  • Master’s Degree in Social Work or counseling related field preferred
