Summary:

JOB SUMMARY:

The Population Health Data Specialist is responsible for researching, documenting, and analyzing multi-faceted data with community health, statistical, financial, and clinical emphasis. Identifies data trends, develops programs to solve problems, and aids in presenting and implementing recommendations and solutions to enhance community health.

A. Experience, Knowledge and Skill

1. Specialized or Technical Education Required: BA/BS degree is required. A Master’s Degree or Project Management Certification is preferred.

2. Core Competencies Required: Excellent written, verbal, communication and

presentation skills required. Strong organizational and financial backgrounds are required. Strong analytical skills and attention to detail required.

3. Previous Healthcare Leadership Experience Preferred: Prior management experience within a healthcare setting preferred.

4. Manual or Physical Skills Required: Must be capable of performing all skills required in day-to-day operations.

B. Work Complexities:

1. Complexity and Difficulty of Work:

The individual performing in this position must be able to coordinate activities and organize work in an efficient and timely manner. The individual performing in this position must be able to evaluate and establish priorities, design and accomplish related goals and objectives and efficiently coordinate the activities and administrative functions of multiple and different operating areas.

2. Seriousness of Errors:

Errors in performance of the responsibilities listed above and/or communications/directives could result in unacceptable levels of operation (including sub-standard service-related performance, sub-standard financial performance, non-compliance with applicable laws and regulations, non-compliance with other external standards, and non-compliance with internal policies, procedures, standards and expectations).

C. Working Conditions:

1. Hazards:

Sensitivity to appropriate precautions and procedures related to personal protective equipment and procedures is required. Exposure to blood-borne pathogens is possible. General knowledge and awareness of work-site and clinical safety and hazards precautions is required.

Traveling in various types of inclement weather is possible. Work-site safety and protective precautions are required.

High level contact with external organizations and their various representatives, members of the general public, local and state government officials and professional associations is required.

E. Additional Responsibilities:

1. Responsibility for Safety of Others:

The individual performing in this position is responsible for the general safety of all personnel and patients/visitors within assigned areas of responsibility. The individual is responsible for maintaining all applicable safety standards within assigned areas of responsibility.

The individual performing in this position is responsible for all supplies/equipment that are utilized within all assigned areas of responsibility.

 

3. Responsibility for Confidential Information:

The individual performing in this position has direct access to patient medical record information and employee/employer demographic/financial and medical information, all of which must be maintained in the strictest confidence. This individual is also responsible for ensuring that employees within the assigned areas of responsibility maintain confidentiality standards as required by NOHS policy.

4. Responsibility for Performance of Work without Immediate Supervision:

The individual performing in this position must perform all position responsibilities without direct supervision and must be capable of assuming new responsibilities and functioning independently within the scope of authority.

Responsibilities:

ESSENTIAL DUTIES:

1. Building reports including (but not limiting to): community health indicators, summary statistics, data discrepancies/outliers, graphical representation of the data distribution, power and sample size calculation.

2. Responsible for accessing, validating, and querying data from various data repositories using available tools (including Microsoft Office programs, EPIC, payor portals, etc.). Sources of data include, but are not limited to, the clinical, operational, financial information systems for the health system and department level, claims data, and comparative data.

3. Interpret data, analyze results using statistical techniques, and design reports, dashboards, and visualizations of numerous datasets for various departmental/strategic functions.

4. Collaborate with Leadership to obtain and maintain sources and mapping tables. Ensure data integrity and data quality, regularly review data, identify trends, and communicates changes.

5. Evaluate data and reports to assess if reporting meets end user requirements. Gather and compile documentation of metrics and data sources to facilitate replication.

6. In collaboration with Leadership, determine project data needs and assist in designing data collection tools and reports as necessary.

7. Prepares reports, proposals and presentations.

8. Performs data analysis to identify patterns in community health disparities, barriers to patient care, healthcare utilization, clinical care outcomes, or others areas of interest.

9. Collaborates with various teams/stakeholders to implement and evaluate improvements.

10. Executes and leads project plans ensuring completion within the defined deadlines, scopes, and budgets.

11. Establishes project goals and manages the achievement within established deadlines.

12. Serves as a point of contact for teams when multiple units are assigned the same project to ensure team actions remain in synergy.

13. Communicates with senior leadership regularly on updates and alignment with strategic goals.

14. Measures project performance using appropriate tools/techniques, tracks completion of action items, and develops baseline metrics of success.

OTHER DUTIES

1. Attends, participates in, and facilitates meetings as requested

2. Ensures compliance with that all North Oaks Health System and North Oaks Physician Group policies and procedures, compliance programs, and federal and state regulatory guidelines.
