RN Care Manager: Seeking CM preferably with previous care management experience in an integrated primary care setting. This position requires a compassionate, clinically strong RN to manage and advocate for our high-risk patients and provide longitudinal and episodic care management with working knowledge of discharge planning and resources, chronic disease management and triage with an ability to guide patients in self-management and adherence to individualized plan of care.
