Become a part of our team of competent and compassionate professionals as a Community Health Worker (CHW) in the Holyoke F.I.T (Families in Transition) grant program at Behavioral Health Network!  Holyoke F.I.T (Families in Transition) is a grant funded program designed to provide wrap-around, outreach, team based care to eligible families through Holyoke Public Schools.  The goal of this program is to help families in need achieve a greater level of health, wellness and stability.  We provide supportive outreach services to assist families in navigating difficult systems and in connecting with relevant services in the community.

The Holyoke F.I.T (Families in Transition) grant program is an innovative model of care, working with families, Holyoke Public Schools and community providers to address the whole health needs of the families in our culturally diverse community.  Holyoke F.I.T (Families in Transition) is a family and person-centered model which focuses on collaborative goal setting to address unmet housing insecurities, medical, behavioral mental and social needs.  Our team consists of a Clinical Care Manager (CCM) and Community Health Worker (CHW) who will meet together with families to assess their strengths and needs and develop with the family a Family Care Plan to address needs and encourage strengths.  Families will receive guidance and support from CCM and CHW on implementing the Family Care plan which will include identifying and maximizing connections with other community resources and supports. Examples of community resources and supports include:

  • Housing
  • Provide short term therapy along with referrals to behavioral health clinics.
  • Referrals to primary care doctors and specialty providers
  • Connecting with the school system
  • Entitlements
  • Transportation solutions and resources
  • Skill building
  • Referrals for other services

Community Health Workers at Behavioral Health Network have the following qualities:

  • High school diploma; Bachelor’s degree preferred.
  • 1-2 years of work experience in a related mental or behavioral health field (required).
  • Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management (required).
  • Ability to work well in a team setting (required).
  • Display empathic skills and the ability to establish rapport with individuals served, family members, and service providers (required).
  • Familiarity with Internet, email systems, and electronic health records (EHRs) (required)
  • Professional demeanor and phone skills in working with persons served, team members, and community collaterals (required).
  • Bilingual in Spanish and English (preferred).
