Position Summary/Essential Duties: 
 
  • Processes transcribed reports: updates and distributes reports as needed, monitors errors and other problems to ensure document integrity.
  • Assembles & analyzes inpatient discharges according to established procedures & meets quality/quantity standards. 
  • Maintains the physician chart completion room, assisting physicians with completion of incomplete charts.
  • Monitors and manages the physician suspension process.
  • Updates appropriate database elements in Soarian as needed: assigns new MRNs, updates changes to the name, sex and date of birth and seeks assistance from senior co-workers or supervisor when needed.             
  • Verifies admission and discharge dates for insurance purposes,
  • Works with Case Mix to research missing discharges on the weekly priority and unbilled list. 
  • Coordinates third party audits
  • Accurately and timely processes Certificates of Live Birth, updates newborn names and responds to related inquiries.
  • Maintains accurate productivity reports and forwards to management on a weekly basis.
  • Processes requests for the release/copy of medical information in a timely and accurate manner, and according to established procedure.
  • Researches unidentified fetal monitor strips, and follows-up to ensure they are assigned to the appropriate patient.
  • Responds to patient record requests and inquires in a courteous, timely and confidential manner.
  • Abstracts Facility Worksheet completely and accurately. 
  • Assists with the training of new employees
  • Provides departmental cross coverage as necessary.
  • Answers phones and processes physical chart retrieval requests.
  • Works with physical charts/documents as needed: Accurately processes, tracks and files patient charts, and ensures META applications are properly updated, files paper documents in physical charts, purges physical records to storage, picks up discharge paperwork up from divisions, searches for discharges not received, processes loose documents received for scanning, creates chart covers as needed, and other various duties.
  • Destroys scanned documents after appropriate retention period.
  • Performs indexing and QA checks on scanned images in OnBase according to established guidelines/standards.
  • Preps and scans documents into OnBase as needed according to established guidelines/standards. 

 *This role may encounter Protected Health Information (PHI) as part of regular responsibilities.  UH employees must abide by all requirements to safely and securely maintain PHI for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Education/Expertise: 

High School Diploma or GED required.  Some college preferred. 
Minimum one (1) year office work experience required.
Experience working with patient medical records, or in a medical setting, preferred.   
Must be organized and detail-oriented. 
Must have good verbal and written communication skills.
Ability to work productively and solve problems.
Ability to work well independently and with all co-workers.


Special Skills & Equipment Knowledge: 
Demonstrated ability to use PCs, Microsoft Office suite (Outlook, Word and Excel), and general office equipment (i.e., printers, copy machine, FAX machine, etc.) required. Knowledge of Soarian, META, IDX, Spheris, Eclypsis or other EMR preferred
