#18-3 Housing Coordinator (HPS1)
Full time opening in our HIV/AIDS Case Management Program.
Position Description: Health Program Specialist
• The HIV Housing Coordinator assists chronically homeless and unstably housed HIV positive clients in location, securing and maintaining quality housing.
• Works with HIV positive clients facing homelessness, shut off notices, eviction notices, voucher programs, and those seeking grant supportive services.
• Actively advocates and assists clients with housing plans to secure housing that meets the client’s needs.
• Coordinates with SRHD Care Management team, city and state housing programs, service providers, and coalitions to increase housing opportunities for clients.
• May need to seek legal advice and support for clients with extreme housing barriers.
Minimum Qualifications:
Required Education/Experience:
• A Bachelor’s Degree in Social Work, Public Health, Psychology, Counseling, or closely related field.
• Two years of previous assessment and/or counseling experience working with the targeted population or related experience in a similar capacity, with some knowledge of the needs, concerns and attitudes of the targeted population.
• Must have evidenced experience or knowledge relating to working with low-income and marginalized populations.
Required Licenses:
• Ownership or/access to a motor vehicle is necessary for this position. A valid motor vehicle operator’s license, and current auto insurance is required to complete essential functions of this position
Required Immunizations:
• MMR (Measles, Mumps, Rubella)
• Tdap (Tetanus, Diphtheria, Pertussis)
• Varicella (Chicken Pox)
• Hep B
• Influenza
Preferred Experience:
• Three years of experience in social services work or other applicable experience is preferred.
