Position Summary/Essential Duties:

UHMG Orthopaedics RN Patient Navigators provide high-touch customer service to Orthopaedic customers, building strong relationships while helping them navigate the healthcare system

  1. Coordination of care and access of specialty and general medicine care and screening. 
  2. Community resource referral, patient education, and navigation/entry point in to UH.
  3. Actively participates in the process for operational improvement and quality improvement.
  4. Involved in learning and self-improvement.
  5. Provides leadership through communication, education and management.  
  6. Provides advocacy for the patient and family.
  7. Tracks interventions and outcomes of patients and families who access UH.
  8. Initiates and maintains positive relationships with co-workers and physicians
  9. Demonstrates commitment to the mission, vision and values of UH.
  10. Serves as a department resource for education and service promotion within the patient and referring provider communities.

Serves as a liaison for referral and care coordination efforts with industry partners that drive value based care initiatives.

Education/Expertise: Bachelor of Science in Nursing required from accredited program

 

Required Credentials, License and/or Certifications:  Registered nurse with current license to practice in the State of Ohio

 

Experience & Knowledge:

  • At least 5-7 years in clinical medicine
  • Fluent in English both verbally and written
  • Comprehensive understanding of System and community resources and requirements for access.
  • High integrity level and ability to maintain confidentiality of sensitive data
  • Experience working multiple healthcare data sources that include Electronic Medical Record application(s), registration and scheduling systems, and medical claims systems.
  • Experience facilitating telephonic patient outreach.
  • Understanding of healthcare industry trends and Federal and Commercial insurance
  • Understanding of System and comments resources and requirements for patient access
  • Comprehensive understanding of various disease processes, and requirements and challenges of self-care.

 

 

 

 

 

Special Skills & Equipment Knowledge:

  • Excellent verbal and written communication skills.
  • Proven time management and organizational skills
  • Demonstrated excellence in customer service with all stakeholders (patient, employer staff, providers, etc…).
  • Demonstrated ability to apply concepts, utilize sound judgment and work independently.
  • Ability to work within a team environment
  • Ability to handle multiple initiatives and priorities within a fast-paced environment.
