Chronic Care Management Nurse, Full time Licensed Practical Nurse (LPN) to work primarily telephonically with identified population to assist patients in the management of their chronic health conditions. Our medical staff works with patients to establish health and wellness goals, identify gaps in care and reduce risk of ER over-utilization and hospital readmission's. Must be in good standing with Board of Nursing. Nursing experience, good computer skills and the ability to utilize mobile technology required.
