Geriatric Support Services Coordinator (GSSC) Full-Time: Responsible for functioning as a member of the Primary Care Team (PCT) in an entity contracted with MassHealth as a Senior Care Option (SCO). To assess the need and eligibility of applicants and clients for enrollment in the SCO programs. To make recommendations, coordinate, implement, and monitor contracted and community services for the purpose of promoting the client's overall health, independence and to prevent premature institutionalization.

Required: A social worker employed by an ASAP and licensed by the Commonwealth of MA, with annual continuing education units in geriatric practice; or have at least a bachelor degree and two years professional experience in the care of persons over the age of 65, with at least one year involving work in a setting where persons over 65 receive health-care services (such as a hospital, nursing facility, a community health agency, or interdisciplinary program within an ASAP); and have the knowledge and skills necessary to work successfully in the interdisciplinary team and to provide or arrange for community long term care services and supports.

Care Coordinator Full-Time: (Care Alliance of Western Massachusetts) engages in activities designed toward eliminating barriers and improving the conditions and well-being and cultural needs of enrollees. The Care Coordinator is an integral component of the community care and long-term care management system for enrollees. They engage in activities, which incorporate a variety of disciplines and funding sources. The Care Coordinator is directly involved in the processes of outreach to assigned enrollees, assessment, long term services and supports care planning and coordination, care transitions, health and wellness coaching, care team participation, connecting enrollee's with social services and community resources, advocacy, problem solving, and gatekeeping. Bachelor's Degree in social work, human services, nursing, psychology, sociology, or a related field from an accredited college. Bi-lingual preferred.

Job Developer (Part-time): Highly organized, detail-oriented individual to promote the Senior Community Service Employment Program (SCSEP). The successful candidate will have experience in job development or related field. Recruit, interview and enroll eligible candidates to the program. Conduct outreach activities throughout the county. Work closely with area businesses to promote the SCSEP program and permanent employment for the participants. This individual must possess strong communication and excellent computer skills and have the ability to function effectively within a team. Bachelor Degree Preferred. Must have reliable transportation. Travel throughout Massachusetts is required.
