BH Liaison

SCOPE OF ROLE

Reporting to the Supervisor, BH Program Liaison, the BH Liaison will play a key role in the development and execution of coordinating and providing direct service, quality assurance and accountability for the agency’s work with intensive care coordination of numerous community-based services. You will function as an integrated member of the BH Liaison team, serving Lake County clients in need of intensive care coordination as they transition between levels of care and navigate multiple systems.  

 

HOW YOU’LL SUCCEED ____________________________________________________________________

  • Provide immediate support for high-risk clients entering the behavioral health system or returning to community-based care from out of home placement. 
  • Extensive and pro-active outreach to clients in the community, including visitation at the client home, hospital, treatment facility or jail, in order to engage clients in successful return to community, including:

 

Hospital Liaison Program

  • Meet with client at state or private hospitals throughout NE Ohio to assess needs and barrier prior to discharge
  • Assist with applications and referrals with the clients prior to discharge
  • Assist with obtaining hospital records and providing these to community-based agency. 
  • Transport clients from hospital to home or community agency appointment upon discharge to assure warm handoff
  • Provide temporary case management services until client is linked with permanent case manager
  • Attend medical, behavioral health, or resource appointments with client
  • Conduct announced and, as indicated, unannounced home visits to monitor and engage client
  • Assist with client access to MSA funds
  • Attend monthly MSA meeting

 

Jail Liaison Program

  • Meet with clients at the Lake County Jail to assess needs and barriers prior to release
  • Transport clients from jail to community agency on day of release or other scheduled appointments to assist with engagement and transition.
  • Communicate with probation and corrections staff as needed to coordinate
  • Assist with any referrals or application prior to release
  • Attend monthly criminal diversion meetings if appropriate

 

SOR/alcohol Detox Liaison Program

  • Meet with client at Windsor-Laurelwood to assess needs and barriers prior to discharge
  • Assist with any community paperwork necessary
  • Assist with linkage to MAT services to assure continuity of care to outpatient med management
  • Transport client to community agency on day of discharge or other dates as appropriate

 

Forensic Liaison Program

  • Act as a forensic case manager for Lake County NGRI clients
  • Monitor client compliance with community plan in conjunction with assigned forensic monitor, with emphasis on linkage, referral, coordinating with other agencies providing care.

 

  • Engage in advocacy and linkage to community resources, including housing, entitlement programs, foodbanks, and specialized medical/behavioral health care based on client preferences and needs, with goal of reducing barriers to care.
  • Attend staffings and community/team meetings to provide input on client needs and barriers.
  • Identify opportunities for accessing wraparound and multi-system funds to assist with meeting stated client goals.
  • Responsible for communicating with partner agencies and other local government entities; building and maintaining strategic relations, alliances and coalitions; developing and implementing strategic initiatives, outreach and education of services.
  • Participates in all quality assurance and utilization review activities.
  • Comply with all agency policies and procedures.
  • Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.
  • May be required to perform telehealth services as determined by Signature Health.
  • Adherence and completion of compliance training provided by Signature Health.
  • Responsible for and completes all productivity/documentation requirements.
  • Participate in all assigned staff meetings and staff development programs.
  • Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.
  • Other duties as assigned.

 

KNOWLEDGE & EXPERIENCE ______________________________________________________________

  • Minimum Bachelor Degree in a human services field required.
  • 5+ years of previous case management or relevant experience required.
  • CDCA preferred.
  • Valid unencumbered Ohio driver’s license and proof of driver’s insurance required.
  • American Heart Association (AHA) Basic Life Support (BLS) certification required.
  • Strong computer skills with Microsoft, Excel, etc.
  • Knowledge of the community, community organizations, and community resources.
  • Knowledge of ASAM levels of care, ability to assist client in obtaining residential, detox, and supported housing services in the community.

WORKING CONDITIONS ___________________________________________________________________

  • Work is normally performed in a typical interior/office/clinical work environment and/or in interior community-based settings.
  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
  • Requires periods of sitting, standing, telephone, and computer work. 
  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
  • Possible exposure to blood borne pathogens while performing job duties.
  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
  • Sufficient dexterity to operate a PC and other office equipment.
