Qualifications
Experience:

• A licensed RN, in good standing, issued by the NJ Board of Nursing
• Graduate from an accredited college with a Bachelor of Science degree with a major in nursing or other health related field, with three years of professional experience as a RN in community health and/or home health
• In lieu of a BS degree, may have 3 years of professional experience as a RN in community health and/or home health, with progressive nursing responsibilities
• Qualifications shall include experience in providing nursing services in a patient’s home environment, inclusive of an assisted living facility.
• Minimum 3 years professional experience (Required)
• Home Healthcare Background (Required)
• Previous experience working with older adult population (Required)
• Comfortability with electronic documentation
• Ability to communicate effectively verbally and in writing
• Strong clinical judgment and critical thinking skills

License:

• NJ, RN in good standing (Required)

Full Job Description
Laurel Circle is a Life Plan community with both independent and healthcare settings. Through the Health & Wellness Navigation Program and the Laurel Circle Help at Home Program, the Independent Living RN will work with residents to help maximize their independence and quality of life with community programs that encompass wellness services and home care services. The Independent Living Health & Wellness Navigation Registered Nurse (IL Navigation Nurse) is responsible for assisting with care coordination, care management, and transitions, as needed. The IL Navigation nurse will provide emergency nurse response visits during work hours and incident and risk assessments The IL Navigation nurse will coordinate and facilitate medication management for participating residents and perform Laurel Circle Help at Home home care assessments for participating residents in required timeframes, develop, implement, and document care plans, and effectively guide and instruct CHHA’s with resident care plans and resident well-being and act as the Laurel Circle Help at Home DON. The IL Navigation Nurse will work with the interdisciplinary and health center teams, communicate timely and effectively with physicians, pharmacies, residents, and families to provide quality care and services to the residents.

Pay: $36.00 - $38.00 per hour
