The Coordinator Patient Navigator accompanies the patient throughout the course of the injury. The course includes both outpatient visits in the clinic, as well as when hospitalized on an inpatient/surgery unit for diagnosis, treatment, symptom management or palliative care. This position is also responsible for care coordination for the patient to increase access to care within the community. As part of their role, the patient navigator will help the patient to make follow-up appointments, ensure proper referrals are in place for patient care, and help navigate the insurance system (precertification, etc). The patient coordinator/navigator is the bridge which links the patient to the health care professionals involved in the patient’s care, as well as with the various necessary resources outside the hospital, such as home health care, physical/occupational therapy, obtaining DME, etc. The Patient Coordinator/Navigator is a resource to the patient, referring organization, and other health care professionals. He/she works in close collaboration with the treating physicians and other team members to ensure all research protocols are followed, necessary studies and labs are performed according to protocol, and to improve overall access and feasibility of patient to receive complete and comprehensive care within our system.

 Responsibilities:

 

  • Coordinates the patient care process including needs assessments, admissions, discharges and follow-up across the continuum of care.
  • Obtains required pre-certification for evaluation and treatment of the patient’s condition (including referrals, labs, DME, physical therapy, home health, and radiology as needed as well as any other needs related to the patient’s injury).
  • Maintains continuous communication with the referring physicians, team physicians, staff members, case managers and any other necessary healthcare personnel within the institution or the community.
  • Completes all related paperwork needed to be submitted to referring organizations as well as obtain and send related medical records as requested.
  • Schedule imagining and appointments related to patient care plan as directed by physician.
  • Acts as support and helps the patient navigate/move through the health care system.
  • All other duties as assigned.

 

 What Qualifications You Must Have:

  • Licensure/Certification/Registration: 
  • Education: Bachelor’s degree in Public Health, Biology, Psychology, Education, Social Work or equivalent degree preferred. High school diploma required.
  • Experience: 3 years’ experience in healthcare, experience in pre-certification process and referral process
  • Special qualifications: Must be able to successfully complete established competencies for the position within designated probationary period. Demonstrated patient teaching skills and compliance with protocol management of patients.
  • Work schedule: Cancer Center Clinic - Full-time - Monday-Friday(8:00 am-4:30 pm)
